HOME HEALTH SERVICES

Our Home Health staff provides high quality patient care in the comfort of the home setting. Registered Nurses are available 24 hours a day, seven days a week.

PMH Home Health offers a wide range of service options, including skilled patient assessment, observation, evaluation, and treatment; therapy services, including Physical, Occupational, and Speech rehabilitation; Social Work assessment, intervention, and ongoing support; Personal Care Assistance; Medication Management and Supervision; specialized education on dietary, cardiac, diabetic, ostomy, and wound care management; Home IV Infusion Therapy; and Home Photo Therapy for infants.

Following a referral from a physician, family member, or friend, our staff works with the patient’s physician to coordinate an appropriate care plan. Patients may also self-refer, and all ages are welcome. PMH’s Home Health program is Medicare and Medicaid certified.
