HEALTH INFORMATION MANAGEMENT

The staff of the Health Information Management Department are providers and protectors of information following State and Federal Guidelines.

Information requests from physicians, insurance companies, attorneys or individuals are performed with copying costs that vary from year to year following State guidelines.

Birth registrations are performed with requests for Social Security Cards if parents so desire at birth. Information is available through the HIM Department regarding all aspects of the birth certificate application process including paternity affidavits.

Department hours are 8 am to 4:30 pm Monday through Friday.
