DISCHARGE PLANNING

The Social Services Department develops effective, efficient discharge plans for patients of Prosser Memorial and the residents of the Transitional and Long Term Care unit to appropriate levels of care. Their goal is to attain the highest practicable physical, mental and psychosocial well being for patients and residents.

As part of the discharge process, PMH Social Services serves our patients as a conduit of care networking with physicians, staff, community services of ALTC, DSHS, home health, Hospice, assisted living facilities, nursing homes, adult family homes, other area hospitals and their employees, as well as the employees of Prosser Memorial Hospital. Our social services team provides a social work program that meets the medically related social and emotional need of patients of Prosser Memorial and the residents of the Transitional and Long Term Care unit.

Services are provided Monday through Friday between 8 am and 4:30 pm. The Department is staffed with one full time and one part time social worker.

Contracted services for crisis intervention are provided with Tri-Cities Chaplaincy on a 24-hour on-call basis.
